   Enter your "Barcode Number" (called the "Juror Identification Number" located near the bar code on your questionnaire or summons).
   Enter your date of birth. (MM/DD/YYYY).
   Click SIGN IN button one time only.

If you are unable to fill out the questionnaire on-line, please complete and submit the questionnaire by mail.
